MEDICAL ELIGIBILITY FOR ATHLETIC PARTICIPATION
In order for our student-athletes to compete on any of our intercollegiate athletic teams, each individual must complete several forms and submit them using their Teamworks account. No athlete will be allowed to participate until these forms are completed and submitted, which must be done once the dashboard goes live, typically the end of April. Student-athletes may login into their respective Student Center account and complete the forms below.

- Athletics Risk Management Form
- NCAA Concussion
- ImPACT Test
- NCAA Sickle Cell (for first year and new athletes only)
- Most, if not all, student-athletes were tested as a newborn for this trait. Documentation of the trait, either positive or negative, must be submitted. If you do not have your newborn result, your primary care physician can order a blood test.

Physicals: All first year and junior student-athletes are required to have a complete physical exam by their home care provider. The Physical Form must be uploaded through your Teamworks.
It is important to note that physicals for first year student-athletes must be completed within six months from the date of their first allowable day of practice per the NCAA.
